The University of Missouri is reporting the following where the wrong items are attached to an instance:
"Good afternoon,
One of our libraries reported the following issue.
"UMSYS received a request on 7/26 from Stephen’s College for the following item:
Title: Treatment of post surgical adhesions : proceedings of the First International Symposium for the Treatment of Post Surgical Adhesions, held in Phoenix, Arizona, September 15-17, 1989 / editors, Gere S. diZerega [and others]. (Instance UUID: 418ca903-bd32-554e-85b9-4c2bb1d1bbeb)
Item barcode: 30120310704 (Item UUID 84c0d5c7-a2c4-5005-a78d-49521f5b8e19)
UMSYS pulled this item and sent it to Stephen’s College. This week they received the book with a note indicating that the wrong title was sent. The request that the Stephen’s college placed was for the title Of human bondage (Instance UUID: 0e0d1036-7506-546d-a49b-9ed2b611420a).
Apparently, a virtual item was created for the title Of human bondage with the barcode linked to Treatment of post surgical adhesions (item barcode: 30120310704). The hold slip for Stephen’s College and the pick slip from UMSYS is attached.
Let me know if you need any additional identifying information for these titles or barcodes.
Stephen’s College has a place a new hold with correct title and barcode."

"Something similar happened this morning. Our patron requested the title The Women/Kristin Hannah, barcode: 220-002106919. We see this title and barcode on the Request record. However, the title of the item we received was Women in 1972 https://openrs.searchmobius.org/instances/2c32ef47-2a71-45fd-a9ac-b50c4a659839"
