Request moved through states without LMS action

Description

In the DCB staging environment

Problem

Ran into a significant problem with a request between St. Louis (borrower) and Sprinfield-Greene (supplier). All went well until the item was loaned to the borrower. About 15 minutes after the DCB request transitioned to "LOANED", it transitioned to "RETURN_TRANSIT" without any action performed in either the borrowing or supplying agencies. There was an attempt by DCB to perform a checkout in the Sierra supplying library by changing the status to "-" but the hold remains in Sierra.
